Social Prescribing is a way in which individuals who are utilizing the emergency room or consulting their General Practitioner (or other healthcare professionals) for issues generally considered to be social issues, to receive non-medical supports in the community.

Why Social Prescribing?  This supports individuals in the areas of physical activity, nutrition, and social engagement as a way of taking control of their own health and enabling them to engage in and contribute to their community.

Those who may be eligible include seniors in the Chilliwack and Hope areas who may be experiencing:

  • Social isolation
  • Major life events, such as the loss of a spouse
  • Physical frailty and/or inactivity
  • Poor nutrition and/or food security concerns
  • Frequent use of primary healthcare or the Emergency Room
  • Poor health outcomes associated with social determinants of health, such as low-income

We are funded by the Government of BC and managed by the United Way.
